Required Academic Forms

  1. Course Withdrawal Form [Download]
    Used by students who wish to withdraw from one or more courses due to medical, personal, or unavoidable circumstances within the prescribed deadline.
  2. Term Drop or Academic Hold Form [Download]
    Used by students who intend to discontinue their studies temporarily for a semester because of personal, medical, financial, or other valid reasons.
  3. Repeat and Retake of Course(s) Form [Download]
    Used by students who wish to repeat or retake courses to improve their academic performance or fulfill degree requirements according to university policies.
  4. Retake of Missed Examinations Form [Download]
    Used by students who were unable to attend scheduled examinations due to legitimate reasons and seek approval to sit for a make-up examination.
  5. Grade Appeal Form [Download]
    Used by students who wish to request a review of their final grades if they believe there has been an error in grading, calculation, or recording of results.
