Used by students who wish to repeat or retake courses to improve their academic performance or fulfill degree requirements according to university policies. - Retake of Missed Examinations Form [Download]
Used by students who were unable to attend scheduled examinations due to legitimate reasons and seek approval to sit for a make-up examination. - Grade Appeal Form [Download]
Used by students who wish to request a review of their final grades if they believe there has been an error in grading, calculation, or recording of results.
